Parent and Student Update Forms—due June 2, 2023
Important Change for 2023-2024: SchoolDoc is the new portal for medical/health/emergency and release forms—replacing Magnus
The 2023-2024 forms to update family contact information are available online. This is your avenue for providing CA with critical contact information for the 2023-2024 school year that will be used for ongoing school communication, for the flow of information with your child’s teachers and coaches, in the event of a health or other emergency, and much more.
You can access the Parent and Student Profile Update Forms from May 1 through June 2, 2023, using your regular CA website login.
- Log in to the CA website.
- Click on your name in the upper right-hand corner.
- Select “Files and Forms” from the drop-down menu.
- Access, complete, and submit Parent and Student Profile forms.
At any time, once you are logged in to the CA website, you can click on the link in the yellow message bar that states, You have 1 (or more) Forms to Review, and you can easily see what forms are there for you to complete.
Forms will be prepopulated with the information that CA currently has in our database. It should only take a few minutes for you to confirm and/or update data for each student and parent. If your child has another parent/guardian living at a DIFFERENT address, that parent should log into their own account and verify their own information.
Separate Email Addresses for Each Parent
Please provide a separate email address for each parent. If you list the same email or household email for each parent record, you will receive duplicate notices from CA.
Moving This Summer?
If you are planning to move over the summer, complete the forms with your current information. When you have your new address, please email: email@example.com or call Chris Barnard at 303-914-2510.
On Thursday, December 21 Colorado Academy will host Grandparents Day on the CA campus, when grandparents come to campus for special programs and to spend time with grandchildren in the classroom. Providing grandparent addresses and emails allows us to send grandparents invitations to Grandparents Day and other CA events, as well as sending them our electronic newsletter. If the Grandparent listed on the Profile Form is now deceased, please let us know, by sending an email to: firstname.lastname@example.org.
Sign and Submit
Remember to enter your electronic signature and submit each form after you have reviewed it. You will receive a confirmation email when you have successfully submitted your form.
Reserve your CA Directory and Confirm Listings—Due June 2, 2023
If you would prefer any of your contact information (phone numbers, email, etc.) NOT be listed in the school directory, follow the link “Do Not Publish in 2023-2024 CA Directory” on the profile forms, or click here.
CA directory information will be available to members of the CA community on the password-protected area of our website. In an effort to advance CA’s sustainability efforts, printed directories will be provided only to those who order a directory online, so please make sure you complete the order question on the form.

Student Medical/Emergency/Consent Forms (SchoolDoc)
Due July 14, 2023
Parents: Please note—SchoolDoc is replacing Magnus.
New this year, CA is working with SchoolDoc, a leading provider of web-based SMRs (student medical records) for schools. Parents may complete the information online and submit additional forms, including immunizations, by simply uploading them. SchoolDoc sent an email to all parents on 5/1/23 with a link to create their new account and complete this process.
Students will not be allowed to attend the first day of school if their health profile is incomplete.
